Customer Understanding & Authorization

My Initial below indicates my understanding of the wireless service(s) requested, the terms and the specific rate plans I have selected. Further, if I selected a new phone purchase from Telliss Wireless, I understand that this is my agreement for that purchase. By signing this Understanding, I represent that I am the authorized person allowed to make these changes, charges and/or purchases. I realize that my cellular service may be month-to-month billing and that I may stop this service any time after the 1st paid month. If I have elected a two year subscription plan, I understand that the purchase price of the phone has been reduced. Should I terminate the agreement early before the two year expiration date, Telliss may charge me full price or the difference of the phone. In addition Telliss may charge $240 prorated over the length of the service agreement if the agreement is canceled early. If the phone is returned within 3 days, activation fees may refunded. Any unpaid charges are subject to collection expenses and/or agencies, including reasonable attorney fees. I understand that I am responsible for payment for which monthly invoices will be sent to me unless I have selected to pay by the provided credit card monthly. I further understand that if I select to pay by mailed invoice, I must have a credit card information on file with Telliss and do authorize, by this agreement, to allow Telliss to use this credit card payment within any month that I do not pay the billing invoice directly. I understand that all returned for “non-sufficient-fund” checks are subject to additional fees. 30 day money-back guarantee with certain restrictions. Both parties agree that the venue for litigation will reside in Hamilton County, IN. Telliss has my permission to verify or obtain credit information to be used for determining eligibility. This Understanding does not supersede the Telliss Wireless Terms and Conditions which has been provided to me.
